Take 474 towards Chillicothe. Stay on 474 an it will turn into Route 6 as it crosses over I 74. Go two miles and exit at Hwy 150. Turn right off the ramp onto Hwy 150 and go approximately one half mile to the next intersection (The Shoppes at Grande Prairie on right). Turn right onto Route 91 and go approximately 1 1/2 miles. The Center for Health is just past Hoerr Nursery on the right.
I-74 Traveling West (From East Peoria, Germantown Hills, Metamora, Morton, Washington)
Take I 74 west across the bridge and through town to the Route 6 exit. Take Route 6 towards Chillicothe. Go two miles and exit at Hwy 150 (Kickapoo/War Memorial). Turn right off the ramp onto Hwy 150 and go approximately one half mile to the next intersection (The Shoppes at Grande Prairie on right). Turn right onto Route 91 and go approximately 1 1/2 miles. Hwy 150 (War Memorial) Traveling West
Take the McCluggage bridge and stay on Hwy 150 until you come to the Route 91 intersection. Turn right (north) on Route 91. You will see The Shoppes at Grande Prairie. Follow Route 91 north approximately one and one half miles. The Center for Health is just past Hoerr Nursery on the right.
I-74 Traveling East (From Galesburg)

Route 40 (Knoxville Ave.) Traveling South
Take Route 40 (Knoxville Ave.) south to Route 6. Get on Route 6 towards Peoria. Take the Hwy 150 exit.
